Do you offer your seat to those in need?

It would make me highly uncomfortable to see an elderly person standing and able-bodied people including myself, sitting! I think it's the way we were brought up.
At the same time, people young or old, should behave in a polite manner! There's less unpleasantness all around that way... In India, some seats in buses are specially reserved for old people, a few for ladies and handicapped.
