An official investigation report released on Tuesday called for disciplinary action against multiple officials and employees of a local bus company in response to a tragic bus accident that killed six people in the summer of 2025 in Shenyang, Liaoning province.

The report was released on the official website of the Shenyang emergency management bureau. It details the incident and institutional responsibilities surrounding it.

The accident occurred at around 7:54 pm on June 17 last year at an intersection in the city's Dadong district, the report said. Wang, a Route 126 bus driver for a local bus company, switched from the third lane to the fourth lane to bypass vehicles waiting at a red light, then ran the red light, veered left, and accelerated toward the intersection's southwest corner, where the bus collided with a wall surrounding subway construction work. The bus also struck multiple vehicles and pedestrians; six people later died despite rescue efforts, and 16 others were injured, while 15 motor vehicles, three electric bicycles, and some structures were damaged. Wang was later arrested.

The investigation concluded that the accident was a major work-related safety incident caused by the driver's violation of safe operating procedures and improper handling of the situation, as well as the bus operator's failure to effectively implement workplace safety rules and regulations.

The report recommended that five people from the bus company, including its general manager, surnamed Li, be dealt with by the city's emergency management and transport authorities in accordance with relevant laws and regulations. It also recommended disciplinary action against two officials from a public transport law enforcement division under the city's transport authorities.

In addition, the report said information concerning seven personnel from the city's transport bureau, including one surnamed Zang, had been transferred to the discipline inspection and supervisory authorities for further handling.

The report was published on the official website of the Shenyang emergency management bureau.
